We want your first visit to Smith Therapy Partners to be a great experience and as smooth as possible. To prepare, please download the appropriate paperwork below, the English New Patient Paperwork (English speaker) or the Spanish New Patient Paperwork (Spanish speaker), and the Medical Records forms and bring them in on your first visit.

• New Patient Paperwork can be found within your patient portal. 

Please arrive at our office 10-15 minutes before your first scheduled appointment. Please bring your paperwork, a photo ID, an insurance card, and the prescription for physical therapy.

Please make yourself comfy so that your therapist may get to the site of your injury. We do have changing facilities and a shower available if required. A parent or guardian must accompany any minor patient for their first visit.

If you don't speak English, please bring a translator with you. Our clinics have team members that speak additional languages who can make your stay more pleasant. Please notify our clinic in advance of your arrival if you require language assistance.

What to Expect on my First Visit

During your first physical therapy session, your therapist will ask you a number of questions to determine your diagnosis and present condition. These questions will assist your physical therapist in creating a personalized treatment plan in detail to help get you on the road to recovery for you.
